MTN MOD Design is a pool design and construction company currently operating in Utah. The company was formed after its management's previous business, Form Pools, filed for bankruptcy.
According to numerous consumer reports, Form Pools left dozens of projects unfinished after collecting substantial payments from clients. Many customers reported financial losses in the tens or hundreds of thousands of dollars.
Social Media Practices: Multiple former clients have reported being blocked on social media platforms after attempting to share their experiences or inquire about unfinished projects. This pattern of behavior appears to be part of a strategy to control the company's public image.
Review Management: Documentation suggests that the company has utilized paid review management services to minimize negative feedback on platforms like Google Reviews. Several former clients have reported that their critical reviews were removed shortly after posting, while positive reviews remained visible.
These photos document actual unfinished projects abandoned by Chip Galloway and Joe Loy. Clients paid tens or hundreds of thousands of dollars for these projects, only to be left with dangerous excavations, incomplete structures, and costly repairs.
Form Pools began my pool/backyard project back in April 2021. They abandoned the project and went out of business in July 2023. I currently have been left with an unfinished pool, no pool equipment, a pavilion building that won't pass rough framing inspection, and a loss of thousands of dollars. The review requires a project completion date but the project was never completed.

Form Pools and Spas, Chip Galloway, Stole over $200,000 from our Family. We paid $147K downpayment for our landscaping and $25K for our small pool mid February 2022. We paid every penny of the pool invoices totalling $50K. We paid $27K for plaster that was straight up stolen.

Below are verified reviews from clients who worked with Joe and Chip of MTN MOD Design during their time at Form Pools. These reviews have been collected from public platforms including Yelp, Houzz, and BBB. Many clients have reported that their negative reviews were systematically removed from Google, while others found themselves unable to post on the company's social media after being blocked.
"I was informed by Chip Galloway that his company - Form Pools LLC - would be abandoning my pool and spa project, despite paying all milestone payments on time and in full on my project. The Utah Division of Professional Licensing commenced action to revoke Form's license to operate today (4 January 2023) due to THREE reports of job abandonment in recent weeks."

"I understand Form Pools and Spas is filing bankruptcy. However, my review is for the next person who decides to do business with the next company that Chip Galloway opens. We started a pool contract with him in 2021. As mentioned from Adam in the previous review. Chip also called us and stating he was going to abandon our project because the profit was not there."

"This is a terrible company!! They currently owe us $46,000 and have avoided our calls for 4 months. the work they did on our Gunite has to be redone because of cracking and crumbling. I currently have a big cement hole and a dead yard with huge weeds because the yard is totally torn up and destroyed since September"

"NEVER use any business associated with Chip Galloway. Make sure you pull all lawsuits and fraud reports prior to commencing any business. Mr. Galloway specifically told me that he used my money to fund other projects and that he would be upset if he were in my position. Well the court ruled in my favor and I am owed significant money. Unfortunately the ruling on this scam is not the end, as now Mr. Galloway thinks he can outsmart myself and the the legal system. Let the many criminal charges of multiple individuals begin."

"Do not under any circumstances use this company. Very poor workmanship. Lots of bad reviews on bbb. We spent 180k and they never finished."
Matt S. - September 2022
"We had been working with Chip Galloway since December 2022. When we were ready, we paid for our swim spa in full in April of 2023. We were told by Stephanie that it was ordered April 14. Originally she told us it would be 6-8 weeks, then 8-10 weeks, and after we signed our contract and paid we were told 10-12 weeks. ... I have never worked with a more dishonest company. They basically took our money and ran. We will definitely do more research in the future and will not ever do business with a company associated with Chip Galloway or Stephanie."

"Form Pools and Spas is the worst company my husband and I have ever dealt with. They owe us well over $100,000 but have shown no intention to repay us. They basically strung my husband and I along for over 18 months, took over $100,000 of our money and didnt provide their goods and services. They had always been terrible at communicating, usually taking weeks to get back to us, but when all communication stopped in June 2023, after more broken promises, we got our legal team involved. After ignoring our legal teams requests, our team finally received an email which stated: Form is no longer in business. Have a nice day. The snarky response to have a nice day after knowingly taking and keeping our $100,000 + showed no remorse whatsoever."

"Form Pools is now out of business after abandoning multiple jobs and ripping off several customers including myself. Be advised that the same people that ran Form Pools are now operating as MTN MOD LLC under a different contractors license. MTN MOD LLC's website shows projects that were previously shown as Form Pools and their Houzz.com posting shows a glowing review for a completed project that wouldn't be possible since the contractor license is only current as of 08/01/23 and the company just started this Summer."
Kevin R. - August 2023
"Form pools stole over $200,000 from our family. They damaged a $120,000 engineered wall and left our home with dug up pits and mounds of rocks and dirt. This is the fourth bankruptcy for Chip the *****. ... Stay away from Chip or Joe, they are dishonest to the core. Hopefully, someone reading my review will be spared the same pain we went through."

"We signed a contract to build a pool and re-landscape our backyard in Sept 2021. They demolished our yard in April 2022. They dug the pool and had the gunnite completed by July 2022. Then we were left with no sprinkler system and piles of debris and dirt. In Feb 2023, they told us if we upgraded to pavers instead of concrete for our hardscape they could get the work done in 3 days. They deposited the pallets of pavers, cleared the snow into the pool and then it snowed 15" and we didn't see them again until May. In May they did some retaining walls before disappearing again. Finally in June we took the landscaping over and got the project completed on 28 July 2023. [Joe] Loy the only project manager left at Form Pools, told us they may go out of business but they were hopeful that they would be able to honor the contract when they opened their new company. As of today, July 31, 2023, we have discovered that all phone numbers for Form Pools are disconnected, the showroom is empty with a "we are closed" sign on the door and no one has been able to contact them. We have paid them for upgraded plaster, stone facing, and a salt water system which they have obviously not delivered. As per a discussion with [Joe] Loy in June 2023, he and Chip Galloway are opening a new smaller pool company which may be called Mountain Modern Pool or Design.

On April 3, 2025, this consumer information resource received the following cease and desist letter from Jeffery Balls and the legal team representing Joe Loy and MTN MOD Design. Cease and desist letter from Jeffery A. Balls, Parr Brown Gee & Loveless, April 3, 2025
Also, let's note Mr. Balls' misspelling of the word principals in the cease and desist letter.
This website contains factual information based on documented customer experiences. The reviews and information presented here reflect real consumers' experiences with Form Pools and its leadership. While Joe Loy may not have technically been a principal owner of Form Pools, public records show that:
As per a discussion with Joe Loy in June 2023, he and Chip Galloway are opening a new smaller pool company which may be called Mountain Modern Pool or Design."
"The U.S. legal system strongly protects consumers' right to share truthful information about their experiences:
The publication of this cease and desist letter further demonstrates the approach taken by these businesses when consumers share their experiences. Rather than addressing the substantive complaints of the many customers who have lost money, they attempt to silence those who speak out.
Yes. Public records and customer experiences clearly demonstrate the connection. While MTN MOD Design may be registered as a separate legal entity, it shares the same operational leadership with Form Pools. Joe Loy and Chip Galloway were both involved in Form Pools, and now they operate MTN MOD Design. The company showcases the same projects on their website that were previously attributed to Form Pools, and Joe Loy is listed as the principal of MTN MOD, directly tying the two companies together.
Yes. Multiple customers who had projects with Form Pools have confirmed working directly with Joe Loy. Customer reviews and testimonials frequently mention both Chip Galloway and Joe Loy by name. Additionally, Joe Loy's own professional profiles and marketing materials previously highlighted his role with Form Pools before the company was dissolved.
The cease and desist letter attempts to create technical distinctions between legal entities while ignoring the practical reality that the same individuals operated both businesses. This is a common tactic used to distance a new business from the reputation of a previous one. However, consumer protection laws focus on the substantive reality of business operations, not just legal technicalities.
Note that their own attorney couldn't even spell "principals" correctly in their legal letter, referring to "principles" instead, showcasing their extreme thoroughness and professionalism.
Companies that prioritize reputation management over customer satisfaction often resort to legal intimidation when facing negative reviews. Rather than addressing the substantive issues raised by customers (abandoned projects, missing funds, etc.), they attempt to silence critics through legal threats. This approach is typically more cost-effective for them than remedying the underlying problems, but it doesn't serve consumer interests.
Yes. Truthful consumer reviews are protected by both the First Amendment and specific federal legislation like the Consumer Review Fairness Act (15 U.S.C. § 45b). Court precedents including Bose Corp. v. Consumers Union and Jacob v. Bezzant establish strong protections for consumer speech, especially when it serves the public interest by warning about potentially harmful business practices.
Public records show a concerning pattern of business formation and dissolution:
Formed in 2002, administratively dissolved
Principal: Chip Galloway
Formed in 2010, administratively dissolved in 2023
Principal: Melissa Galloway
Formed in 2010, administratively dissolved in 2015
Principal: Melissa Galloway
Formed in 2021, administratively dissolved in 2023
Principal: Melissa Galloway
Formed in 2022, currently active
Registered Agent: Form Landscapes, LLC
Formed June 12, 2023, currently active
Principal: Joseph Loy
This pattern of creating new business entities while dissolving previous ones is commonly associated with attempts to escape liability and negative reviews from prior business operations.
