"We had been working with Chip Galloway since December 2022. When we were ready, we paid for our swim spa in full in April of 2023. We were told by Stephanie that it was ordered April 14. Originally she told us it would be 6-8 weeks, then 8-10 weeks, and after we signed our contract and paid we were told 10-12 weeks. ... I have never worked with a more dishonest company. They basically took our money and ran. We will definitely do more research in the future and will not ever do business with a company associated with Chip Galloway or Stephanie."

"Form Pools and Spas is the worst company my husband and I have ever dealt with. They owe us well over $100,000 but have shown no intention to repay us. They basically strung my husband and I along for over 18 months, took over $100,000 of our money and didnt provide their goods and services. They had always been terrible at communicating, usually taking weeks to get back to us, but when all communication stopped in June 2023, after more broken promises, we got our legal team involved. After ignoring our legal teams requests, our team finally received an email which stated: Form is no longer in business. Have a nice day. The snarky response to have a nice day after knowingly taking and keeping our $100,000 + showed no remorse whatsoever."

Be advised that the same people that ran Form Pools are now operating as MTN MOD LLC under a different contractors license. MTN MOD LLC's website shows projects that were previously shown as Form Pools and their Houzz.com posting shows a glowing review for a completed project that wouldn't be possible since the contractor license is only current as of 08/01/23 and the company just started this Summer."

This is the fourth bankruptcy for Chip the *****. ... Stay away from Chip or Joe, they are dishonest to the core. Hopefully, someone reading my review will be spared the same pain we went through."

"We signed a contract to build a pool and re-landscape our backyard in Sept 2021. They demolished our yard in April 2022. They dug the pool and had the gunnite completed by July 2022. Then we were left with no sprinkler system and piles of debris and dirt. In Feb 2023, they told us if we upgraded to pavers instead of concrete for our hardscape they could get the work done in 3 days. They deposited the pallets of pavers, cleared the snow into the pool and then it snowed 15" and we didn't see them again until May. In May they did some retaining walls before disappearing again. Finally in June we took the landscaping over and got the project completed on 28 July 2023. [Joe] Loy the only project manager left at Form Pools, told us they may go out of business but they were hopeful that they would be able to honor the contract when they opened their new company. As of today, July 31, 2023, we have discovered that all phone numbers for Form Pools are disconnected, the showroom is empty with a "we are closed" sign on the door and no one has been able to contact them. We have paid them for upgraded plaster, stone facing, and a salt water system which they have obviously not delivered. As per a discussion with [Joe] Loy in June 2023, he and Chip Galloway are opening a new smaller pool company which may be called Mountain Modern Pool or Design.
